Ultimately, the way I ended up fixing the problem as directed by Sony Support was as follows: 1 - Deactivate my wife's account as primary. Not sure if this was necessary, but doesn't hurt unless you're using both to purchase content from PSN. They showed as purchased, just needed to re-download them. Upon loading the game again all the purchased DLC's were available to her. I switched back to my account just to verify it was on mine as well and everything is working as it should again.
I'm going to assume that even though I was able to download the DLC's upon initial setup of my PS4, it did not properly assign the licenses to my PS4 primary account. Most likely because the content was originally purchased via the PS3. Thanks for all the help to those that replied. Hopefully my experience can help other users in the future with this same problem. I am a total Batman fan so I tend to buy and read all of the graphic novels I can get hold of, that I can afford any way! I actually bought the Arkham Knight graphic novel from Amazon. In the back cover there is a redeem code for a skin.
I have attempted to enter this redeem code whist logged into my UK PS4 account but it refuses it say it is invalid. I have established that it is a code that can only be used in the USA. They reckon that this will allow me to enter the redeem code and therefore download the skin. There was no mention of whether it could be read across accounts on the same PS4, but I didn't ask, maybe because I didn't fully understand.
I haven't done it yet as I am not really sure of what I'm doing.
